Apply for recognition as a caregiver

You are a caregiver at heart, but preferably also on paper. Apply here for your recognition as a caregiver or renew it.

Gather the correct documents

Download the declaration on honour for recognition, or the declaration on honour for renewal of recognition.
Complete and sign the declaration.
Gather any supporting documents.
For caregiver leave with social entitlement: also include the certificate of care dependency for the person you care for.

Conditions

General recognition (certificate or caregiver status, does not grant specific rights but can serve as supporting evidence)
- Valid for life
- Applicable to all statuses
Recognition with social entitlement (grants entitlement to specific leave schemes)
- Only if you are employed
- If incapacitated for work or receiving invalidity benefits: can be reviewed with the advising physician
- Not applicable to self-employed persons, unemployed persons, and pensioners.
