Onze maatschappelijk werkers en ergotherapeuten:
- geven algemene info over palliatieve zorg.
- luisteren naar de situatie en bekijkt samen welke dienstverlening het best opstart.
- verwijzen door naar de arts als er nog geen palliatieve thuiszorgequipe is, zodat die de equipe kan aanvragen.
- verkennen jouw rechten: juridisch, financieel en sociaal.
- ondersteunen bij vroegtijdige zorgplanning en wilsverklaringen.
